Medications used for weight loss have the potential to cause serious illness and severe side effects if used inappropriately, and our doctors will not be able to assess your health risks if false or inaccurate information is provided.

Please answer the questions as accurately as possible, and if you're unsure of anything please contact us or ask your doctor. Your treatment outcome will ultimately be a joint decision between you and our prescribing clinician. Before you continue, please check the following important information:

• We ONLY dispatch medication within the United Kingdom.

• Your medication will ONLY be dispatched once the clinician is satisfied that you meet the criteria and clinical safety checks are completed. If you are deemed unsuitable, a full refund will be given.

• Weight loss medications are not a replacement for a healthy lifestyle, and should be used alongside a calorie-controlled diet and increased physical activity.

• Only one type of weight loss medication can be used at the same time.

• If your body mass index is over 40, other treatments like bariatric surgery may be more beneficial. To consider your other options you should speak to your doctor.

• We will need you to upload some photos to check your identity and current weight.

• You will also need to let your GP know that you are using this service.

• You understand that if you develop any lumps in the neck or hoarse voice whilst taking this medication, you should stop treatment and speak to your doctor.

• Weight loss and weight loss treatment have been associated with lowering of mood. If you are experiencing this (depression, thoughts of self harm or other mental issues), you will stop the treatment and speak to your doctor.

Contraception

• GLP-1 injectable weight-loss medication (such as Mounjaro and Wegovy) may reduce the effectiveness of oral contraceptives and we recommend that you use additional non-oral contraception methods (e.g. condoms) during your treatment.

• This medication should not be used by men or women that are either trying to conceive or are within two months of starting to try for a child

Agreement

• You will read the patient information leaflet supplied with your medication

• You will contact us and inform your GP of your medication if you experience any side effects of treatment, if you start new medication or if your medical conditions change during treatment.

• The treatment is solely for your own use

• You understand our prescribers take your answers in good faith and base their prescribing decisions accordingly, and that incorrect information can be hazardous to your health.

• You understand that there may be an increased risk of pancreatitis, gall bladder problems and gall stones with this medication, and if you experience any abdominal pain whilst using this medication you will seek medical advice.

• I agree to a short telephone video consultation (or in person if you prefer) with a qualified clinician.

• I understand that I may not be suitable for treatment and the clinician's decision is final.

• I consent to Lifestyle Medical Clinic Ltd storing my medical history and data that will not be shared with any 3rd parties.

• DO WE NEED TO SAY SOMETHING ABOUT OFF LABEL?

• I understand that I need to allow a 5 week gap before starting a different type of medication (e.g. switching from Wegovy to Mounjaro).
